If you really wish to achieve top shelf lighting on small objects, jewelry, etc. You need to use very small sources. The finest jewelry photographers use fiber-optic systems which essentially allow the same size softbox (or spot) : subject size relationship that one would use for larger objects. You can achieve similar effects by controling the size of soft sources with small holes cut into larger black boards (and diffusion material set in the hole).
