Just a thought,

I  bought a Focotar cheaply once that was soft all over but would just about
focus in the centre. Someone had put the rear element in the wrong way
round.

The 40mm Focotar has been around for a long time. Is it possible that your 
lens is suffering from initial fungal growth? Check it with a bright 
flashlight just in case.

If that's not it, you must either have blurry paper or blurry developer...
;-)

PS... could it be on the neg that way? Could it be your camera lens? Have 
you looked at your negs with a high power loupe, at least 10x ?
