There are no additional adaptors:  the lens heads themselves screw directly
into the adaptors.  Here's a simple chart:

OTZFO/16464     3.5/65 Elmar, 2.8/90 Elmarit, and 4/135 Tele-Elmar
OTZFO/16464 AND OTSRO/16472     4/135 Elmar, 4.5/135 Hektor
OUAGO/16467     4/90 Elmar
ZOOEP/16463     2/90 (1st type) Summicron -- scaled mount (has distance scale)
16462           2/90 (1st type) Summicron and 2.8/135 Elmarit -- unscaled mount

The OUEPO/16474 is a close-focus adaptor to be used with either the 2/90 in
ZOOEP/16463 or 16462 adaptor or the 2.8/135 in 16462 adaptor.  This usage
will yield a 1:4.5 reproduction scale.

Copies of the Leitz charts on this are readily available.  They are
reproduced in Laney's outstanding _Leica Collectors Guide_ and in several of
Gianni Rogliatti's books.
