Regarding the use of Leica M cameras and flash (both electronic and bulbs) the 
most complete reference is 
Guenter Spitzing. 1974. (and later editions) "The photoguide to flash" (Focal 
Press London & New York).  It is also available in Dutch and German and written 
in a competent but very humourous style.  Spitzing's book first introduced me to
the Leica system and this book covers everything from fill-in flash, macro flash
to illuminating huge cathedrals whith several flash heads or bulbs.  The older 
editions (4th) give more room to the use of flash bulbs at faster than 1/50 
shutter speed on Leica M's.

One useful piece of equipment Spitzing mentions is a "mouth shutter release 
cable" supposedly available (in 1974) from Leitz.  Has anyone on this list heard
of this device?  It frees one hand to hold a flash while the camera is fired by 
a springy cable release held between your teeth.  Would love to get one of 
those.
