I've finally settled for a Vivitar 283 mounted in a CTOOM. This not small
but it is powerful and when so mounted is reasonably well balance. The
flash unit tilts and the CTOOM pivots.

Try a Sunpack 383. It's similar to the Vivitar 283, but it pivots and has
more options for control. I think it's a little bit smaller too.
