The adapters mentioned are the following:

16472=OTSRO: adapter to mount the lens head of the Elmar 1:4/135 to the
universal focussing mount OTZFO=16464.

16471=OTRPO: similar to the above, but shorter. This is an extension
tube for macro work with the above setup. it is mounted between the
OTZFO and the OTSRO.

16469=OUFRO: M-mount extension tube. It's intended use is for macro with
the summicron 50 or 35 mounted to the visoflex II/III. allows 1:1 and 1.4:1
respectively. Note that with or without this tube, only macro work is
possible with this setup, as the viso acts as an extension tube itself.

The problem with the ZOOAN sounds weird to me. The ZOOAN should
allow focussing from infinity to 1.5m with the Hektor 4.5/135 head in either
of the following setups:

VISO II + OUBIO + ZOOAN + Hektor lens head

or 

VISO III + OUBIO + ZOOAN + Hektor lens head

or

VISO I + ZOOAN + Hektor lens head.

If it does not, the only reason I can imagine, is that your ZOOAN is not
a ZOOAN but something different.
