Of these, the ones I know are:

35mm Summicron = Canada (early), Germany (late)
50mm Summicron = Germany (early), Canada (1980s+), current ?
60mm Macro-Elmarit = Germany
90mm Summicron = Canada
100mm Apo-Marco-Elmarit = Germany
135mm Elmarit = Germany (early), Canada (late)
180mm Apo-Telyt = Canada

Older, discontinued lenses I know of are:

21mm Super-Angulon-R = Germany (Schneider)
35mm Elmarit-R = Germany (Leica)
90mm Elmarit-R = Germany (Leica)
250mm Telyt-R = Canda (early, Leica), Germany (late, Leica)
