I've had this happen. Get used to using SLRs. Use an M. Forget to focus.
Also had an M with a rangefinder out of adjustment, but you can easily tell
if the problem is in the camera.

I've been using my Rs more lately. Seems to run in cycles. I'll use M
equipment for a time. Then I'll use Rs. The R has several things the M
doesn't. Easier to use a polorizing filter (which I tend to use this time of
year). DOF preview is handy for certain things. The 80-200/4 is a wonderful
and very useful lens. AE is handy. 

Can't say I prefer one system over the other.  But you're right, when you're
in an SLR frame of mind, sometimes it's easy to forget to focus an M. In
general I think the M is more difficult to get used to than an SLR.

I have recently switched back to my M2 after quite some time using
SLR's.

Too many of my M2 fotos are way out of focus.  I can think of only one
reason.  And that is because the rangefinder viewfinder is always in
focus I don't make any correction.  This must be it, because the out of
focus fotos are nearly always among the first on the roll.

This must happen to others especially to folk who carry more than one
camera but I have never seen it mentioned anywhere.
