Hi Noel,
       I am looking the original Leitz Catalog: Gesamtkatalog für den 
Fachhandel (April 1969), page 45: and your adapter is #16596 (with  50mm: 
1,4:1-3,2:1 and 35mm: 2,1:1-4,7:1) to be used with Visoflex II or III and 
bellows. S, Claudio.
