Frank, I think you may be getting the f/5.6 and f/6.8 Telyts mixed up.
Simon's 560 is the f/6.8 model (trombone focus) while the 400mm f/5.6 you
mentioned recently uses the Televit focus grip.  The trombone-focus lesnes,
both f/6.8, don't have the fine-focus knob that the Televit has.  The grip
on the shoulder stock (f/6.8) is for the RIGHT hand, when used with a
shutter release that fits into the hole at the top of the grip.  I prefer to
keep my right hand on the camera body so I don't use the shoulder stock's
grip.
