Alastair,

Lamy Safari Fountain pen, cheap (about $25), matching ballpoint or Lamy 
Accent 
(fountain pen and roller ball; perhaps Recife (not Lamy) roller ball. The 
Accent 
rollerball is almost as good as a fountain pen in the field.

Waterman, Mont Blanc, Cross, Parker I have - or have lost - them all. The 
Lamy 
just works out cheaper and more reliable.

Incidentally the Accent can be customised at no extra cost in most shops 
selling 
them; no vulcanite or hand-worked leather, but I think there is a "titanium" 
finish!!
