Depends. I often have to mix digital and traditional capture.

Capture on film (motion picture or still) -> Film scanner (Imagica etc 
or drum/nikon scan) -> Film scans are
'graded' in Nuke, Shake or Photoshop etc. -> Image manipulated in 
Shake, Nuke, Flame, Photoshop  etc. ->
Output on Arriflex film recorder or other output device

Video (uncompressed (Viper etc) -> load onto network via docking 
station -> Capture is graded in Shake or Nuke ->
image manipulation in Shake, Nuke, Flame etc -> output to Arri film 
recorder or tape.

Digital camera Capture -> frames graded in Nuke, Photoshop, HDR shop, 
Capture One, Nikon capture etc. -> Manipulated in Nuke, Shake, 
Photoshop etc -> Output varies
