Hi Steve,
One thing you do in every child picture is you capture an absolute feeling 
of "a real moment!" Nothing feels set-up or posed.

It's the available light factor you use without fear and the Noctilux 
feeling, whatever that is in ones eyes. It makes your pictures look like a 
real life moment of a child! Which we all know they are whether the subject 
is an ill child or not, they truly are live moments in a real life.

Maybe what I'm trying to say is... "they all look natural."
