Look for a tiny grub screw, and it could be covered  by the adjacent ring.  

May need to unblock the focus block (inside lens) and run out the focus
ring.

Hello,
   
    I do have a 135/4 Tele-Elmar with the aperture ring off index, which
means that actual setting f2 aligns with marking of f8 or so.  I do not know
how it happens as I bought it used.  I do not see any sign of disassembly
though.  Is it a simple adjustment?
