It might help if you shared the serial 
number.  The 4/9cm or 4/90 Elmar was produced in 
a long production run of four-element lenses 
which were superseded in the middle 1960's for a 
short run of three-element lenses using specialty 
glasses.  Without looking it up, I cannot be 
certain, but I recall that the lens head on the 
3-element lens was not removeable while that on the 4-element lens is.

In any event, you ought to be able to use the 
lens directly on your M camera with the M adapter 
without the need for a Visoflex intervention.
