Axial Alignment is critical.  I found using  a lens that was coded correctly 
as a example of placement, made a big difference in my success.

Milich made a LTM-M adapter with milled out recesses that worked like a 
charm.

There are CHinese adapters now out there that have the milled out bits at 
$40 each, which is another solution.
