The cheapest way is to start from a LEICO (as mentioned by W.J.
Markerink) or similar no-name ring made for the use of M42 lenses
on Focomat or similar devices.
Nonames cost about DM 25 in Germany.
To get the thing focused to infinity,
I bought an old M42 macro extension tube on a camera show
(between $0 and $5). Both rings together were only 0.85 mm to
short. to fix this, I added two rings cut from mylar foil (DuPont) 
which is available in 350 micron and 500 micron thickness, between
them. Now the distance is exact by less than 10 microns.
This works fine and was much less than $300.
