I too have often wanted one, last week I saw my first (for sale, it was too 
expensive 250GBP though) and I can confirm it is a straight adaptor, no 
focussing scales, helical cams, etc.
You focus normally with the R lens (by guesstimation) mounted via the 22228 
on the M (or Leicina Special) body.
I suspect that if you desperatly want one then a qualified mechanic could 
build you one more economically than sourcing a genuine 22228.

The 22228 has been mentioned a few times lately. I've heard of them for
many years but have never had a chance to examine one. Is it just an
adapter ring, where you scale focus the R lens, or does it have a focusing
helical and cam?
