John,
I'd second the suggestion to visit Cameraquest, there you'll find the bad   
news, they're very expensive and rare.
Here's a bit of good news, I bought one a week or so ago which wasn't too   
expensive, though it only works with external mount Contax lenses, I   
haven't been able to try out its accuracy yet, as I thought (when I   
bought it) it would mount internal bayonet lenses, and that's all I   
have...
But it looks like it should work well, and yes they do preserve r/f   
coupling.
I also have 3 others which don't offer r/f coupling and I presume they   
were for mounting contax lenses on enlargers/projectors etc.
