Henry,
    The form or forms you will need is Customs form # 4457. "Certificate of
Registration for Personal effect Taken Abroad."
You can get the forms early, and have them filled out before you visit the
proper U.S. Customs office. It only takes a couple of minutes when you have
the forms ready, unless you get a new agent, who wants to look at
everything.
    The forms do not expire, and produce them only when asked.
    In all my travels, I've been asked for them only a couple of times, but
it beats having to pay duty to get your stuff back into the country.
