 if and where I can go to get B&W printed printed with a continues tone?'

For short runs, you could design the whole thing in photoshop and get it
printed digitally. If you duplicate the image, you could get 4 card on to a
piece of 6 X 4 inch paper and get the bureau to guillotine them to size. For
short runs this could be an economic way of getting a continuous tone photo
with type. It also means that you can use dropped shadow, embossing and all
the other goodies that photoshop provides.
