Hi Nelson,

I am not convinced that the problem lies in your technique, since
NPS and Portra 160 work for you, unless you always use different films
for different subjects (say NPS and Portra 160 for close-up portraits,
and NPH for landscapes, or portraits in landscapes).
In any case if the underexposed shots with the wider angles include
areas of sky, you may want to try pointing the camera down when you
meter, to minimize the bias of all that light in the sky.  You may or may
not already know this.

Or use an incident meter.  :-)
