Mark, the caption calls it "I Vow to Thee My Country / Thaxtet / Jupiter".
 In other words, the hymn "Thaxtet" adapted by Holst from the Jupiter
section of The Planets, with lyrics based on a poem by Spring-Rice.  Here's
