One of two people will have this done before you walk in the door. Either 
the surgeon who allows you in the OR to shoot his work.

But most likely the Head Nurse of the Surgical Floor, like the super head 
honch, will have it attached to the patient's chart. And if she doesn't have 
one don't be surprised if you're asked to go sit in the lounge!
