I don't know if this'll be useful to you... it isn't Leica related at 
all and I haven't had a need to use it for a long time:

Nikon PB4 bellows with a Nikon bellows lens, Nikkor-P f4,105mm,
Nikon EM, don't laugh, it works fine for this,
Velbon "Macro Slider" focusing stage,
Sunpak GXBR ring light,
Tiltall Tripod.  (not for sale Jerry) ;-)

I don't know about a 200mm but it might keep you from getting an aching 
back :-)
