Hi there. With the Visoflex 11 and 111 and the Elmar f4 135 head, the OTZFO 
(16464 or 16465) and OTSRO (16472) adaptor will work. I
just tried both on an M6 classic and an M3. You can add the OTRPO (16471) to 
go closer. These are the model numbers I have. There
will be variations. You need the universal focusing attachment first. That 
is the OTZFO, I think that is what you are missing. The
16472 goes into the OTZFO, then the lens head or you can have OTZFO, 16471, 
then 16472 then lens head. 
Other adaptors may be possible.
The Tele Elmar head will work straight into the OTZFO but the other adaptors 
won't fit that one. Those are the only two f4 135's I
know of (I have both). There was a later version of the tele- Elmar. Though 
the optics are the same I don't know if that one has a
detachable head. I think not.  You might look at the bellows option too. 
That gives you more versatility. You need the appropriate
adaptor for it (16558,) originally came with the bellows II.

/f4 unscreweable lenses-head (not TELE-ELMAR or
TELE-ELMARIT ! ! !) on a VISOFLEX II or III ...
16471 OR 16472 ARE NO SUITABLE FOR IT ...
