when tonsillectomy was thought to work, and be necessary, and was funded, 
there were lots of tonsillectomies....

so it goes, but I caution you, it is very much the same in other fields, 
and...

re the book Mark, surprise.....it's a grievous error to suppose that all 
doctors think alike,
