That didn't format right.  It should be Witness on one line (somebody 
who witnesses the signature) and Address on another line.  The 
address is handy if you have to contact the person later to confirm 
or change the model release.  I give them one of my cards and tell 
them to contact me for a print. That fulfills the "valuable 
consideration" part of the release.  Most people never bother to 
contact me to ask for the print; but at the time they sign, it makes 
them feel better about signing the release.
