In your case, Clayton, I would suspect wear and tear on the USB  
connector(s) in your computer and/or your USB cable.

There is an issue with long term USB plug durability. USB connectors  
are only rated for 1500 insertion/removal (according to my hardware  
friend) before wearing out and requiring replacement. 1500 insertions  
doesn't take long if you do it 10 times a day.

[Note: I did go check this USB 2.0 specification just now and that  
number is accurate. Table 6-7 of the USB 2.0 specification if anyone  
is really keen.]

This is why changing the USB cable as a first step is not a bad place  
to start.
