With Salgado, from what I have read it is a combination of who and 
what.   He (and/or assistants) work on the digital file to taste, have 
it written to a 35mm or larger film negative and printed in the darkroom 
on silver paper.  In addition, as Jayanand noted, I have read that the 
Genesis exhibition prints were inkjet.  Somewhere in the mix I think 
platinum/palladium contact prints were made too for collectors.
