UPDATE UPDATE ... Sorry, Eric, I was quoting the figures from 2002. The
new figures are out and the US has fallen from 17th to a tied 32 place
(together with Greece).

So to answer your question with updated information, I will not mention
one but:

Finland, Iceland, Netherlands, Norway, Denmark, Trinidad and Tobago,
Belgium Germany, Sweden, Canada, Latvia, Czech Republic, Estonia,
Slavakia, Switzerland, Austria, Ireland, Lithuania, New Zealand, Slovenia,
Hungary, Jamaica, South Africa, Costa Rica, Uruguay, France, United
Kingdom, Portugal, Benin, Timor-Leste ... and then Greece and the US
